The endorsement of the examination in the booklet and the additional opinion of the prescriber are required. For the directly insured (except the Coast Guard), the MRI needs an additional certificate from the General Hospital of Lamia that the examination is not carried out there.
Directly insured have 0% participation and Members have 20% participation.
